Scan the QR code, open My12 on your phone, then save it like an app so leaders can get back to care, attendance and follow-up quickly.

Scan the QR
Open your iPhone Camera and tap the My12 link when it appears.

Open in Safari
If another browser opens, copy the link and open it in Safari.

Share from Safari
Tap the More button, then Share. If your Safari layout shows the Share button directly, tap that instead.

Add to Home Screen
Choose Add to Home Screen, turn on Open as Web App, then tap Add.

If Add to Home Screen is missing, scroll to the bottom of the Share list, tap Edit Actions, and add it.

Once it is added, My12 opens from the Home Screen like a normal app.

Leaders can log in, see their people, submit attendance and care consistently from their phone.
